Comprehensive Assessment & Care Plan

A RN Care Manager will perform a comprehensive physical, environmental, psychosocial, safety, and medication assessment of your loved one and provide you with a written report and detailed plan of care.

1-Hour Phone Consultation (outside of Houston)

If you, the caregiver, or your loved one is outside of the Houston, TX area, we would be happy to have an RN Care Manager contact you for a 1-hour phone consultation to review the case and advise you on the best course of action.

On-Going Care Management

This service is ideal for loved ones with long distance family support or those elderly in need of additional local support. Regular visits can be provided at home, in assisted living or in skilled nursing facilities. Medication oversight is one of the many important services that can be provided on an on-going basis. Emphasis is placed on communication between your loved one, the RN Care Manager, any providers involved with their medical care, and the family.

Health Advocacy

A RN Care Manager accompanies your loved one to physician appointments. We take written notes for your loved one and family. We can ask specific questions as provided by the loved one, family member of facility/personal care home. This assures that your loved one is being heard and their needs are being met. We provide an e-mail or telephonic report to designated family members or caregivers. We schedule all follow-up appointments, lab work, and assist with prescriptions. If the client is hospitalized, the RN Care Manager will help your loved one navigate the health care system while keeping the family informed.

Assistance With Moving

When your loved one is no longer safe to live at home, a RN Care Manager can assist with finding an assisted living facility or personal care home. We will do a face to face or telephone interview to assess your loved one’s physical, social and financial status. We will then search the database of all facilities in the area that you would like your loved one to live. We will search each facilities license report to assure they are in good standing and do not have any deficiencies. We will contact these facilities to assure they are a good match and have rooms/apartments available and then we will make you a comprehensive list based on these results. We do all the leg work for you. Your search is narrowed significantly so as not to waste your time on facilities that your loved one cannot afford or would not be comfortable in. All that is left for you to do is visit the facilities and decide which one your loved one would like to move in to. This particular service is free of charge as the assisted living facilities and personal care home pay the referral fee. We can also assist you with coordination of movers and other services you may need at this time.

Assisted Living Facility/Personal Care Home Oversight

We can provide a RN Care Manager to any ALF/PCH for medication oversight, caregiver education, admission assessments and many more medically related tasks.
